Kansas City is currently experiencing an unprecedented tuberculosis (TB) outbreak that is impacting two counties. Health officials have warned that transmission remains active and may not yet be contained. As of January 24, 2025, there have been 67 confirmed TB cases, making this the largest outbreak recorded in the history of the United States. Wyandotte County has been the hardest hit, reporting 60 cases, while Johnson County has recorded seven cases, according to Jill Bronaugh, a representative from the Kansas Department of Health and Environment (KDHE).

The outbreak, first identified in 2024, has rapidly escalated, prompting concerns despite health officials assuring that the general public faces a “very low risk.” Bronaugh highlighted the importance of ongoing vigilance, stating, “This outbreak is still ongoing, which means there could be more cases.”

In response to the outbreak, KDHE, local health departments, and the Centers for Disease Control and Prevention (CDC) have joined forces to tackle the situation. Their efforts include monitoring cases, providing treatment to those affected, and implementing preventive measures. While the outbreak has strained local health resources, Kansas has made significant progress in reducing TB cases statewide. Active TB cases have decreased from 109 in 2024 to just one by early 2025, with latent infections dropping from 626 to seven.

KDHE Deputy Secretary Ashley Goss described the outbreak as “unprecedented,” emphasizing its severity during a meeting of the Senate Public Health and Welfare Committee. She noted that, although the situation remains critical, the number of active TB cases in Wyandotte County has decreased from 65 last summer to around 32 at present. The CDC continues to offer on-site support to KDHE in its efforts to manage the outbreak. Goss underscored the importance of timely treatment for TB patients, indicating that individuals with active TB are generally no longer contagious after 10 days of medication and three negative sputum tests. “They can return to their daily routines and work, as long as they adhere to their treatment,” she remarked.

Tuberculosis (TB), caused by the Mycobacterium tuberculosis bacterium, primarily affects the lungs but can also spread to other parts of the body, such as the brain, kidneys, and spine. It exists in two primary forms: latent TB, which involves dormant bacteria that do not cause symptoms or pose a risk of transmission, and active TB, which is contagious and potentially life-threatening.

Symptoms of active TB include a persistent cough lasting three weeks or more, chest pain, coughing up blood or mucus, fatigue, unexplained weight loss, fever, chills, and night sweats. The disease spreads through airborne droplets when an infected person coughs, speaks, or sings, particularly in enclosed, poorly ventilated spaces. The bacteria can remain airborne for several hours, posing a risk to those nearby.

Preventive strategies play a critical role in controlling tuberculosis (TB) outbreaks. Addressing latent TB is essential to prevent its development into the active form. The treatment of active TB requires a prolonged regimen of antibiotics to eliminate the bacteria and reduce the risk of further transmission.

The ongoing outbreak has underscored the importance of early identification, timely intervention, and public education in the management of infectious diseases. Despite significant progress made, health officials remain vigilant in monitoring and responding to the situation, aiming to curtail the spread and mitigate its impact on the community.
